Voluntary Reports

Healthcare professional and consumer may voluntarily submit a report on adverse events or other problems arising from the use of health product of any category under the responsibility of the Thai Food and Safety Administration. Please note that the HPVC only accepts reports covering incidents that happen in Thailand or arise from the use of products manufactured in Thailand. A report of this kind may be submitted through these 3 channels: 

1) through the “AE Online Reporting program” (in cases of healthcare professional reporters) or “Adverse Event Resulting from the Use of Health Product Reporting program for Consumers” (in cases of consumer reporters) on the website: https://hpvcth.fda.moph.go.th/; or

2) email: adr@fda.moph.go.th; or

3) by post or in person at the Strategy and Planning Division, Thai Food and Drug Administration, Ministry of Public Health, Talat Kwan Sub-district, Muang District, Nonthaburi, 11000.
