Procedures

1.

Submit documents of the request for the use of the information system and accompanying evidence at the One Stop Service Center, OSSC Building, Floor 4.

 

1.1 Request Form *Where the requester is a principal
• A copy of business registration certificate of the company or partnership, or a copy of commercial registration certificate (issued no more than 6 months prior, signed and certified by the authorized principal)
• A copy of Thai National ID Card of the principal (valid on the day of submission, signed and certified)

Note: The requester may be allowed to use the information system of the Food and Drug Inspection for no more than 1 year.

*Documents of the request for the use of the information system may be submitted to bie.thaifda@gmail.com in advance to gain a temporary right of use. Once received, the authority will proceed with the request within 2 hours of the working days. (Office hours: 08:30 - 16:30 daily including public holidays)

- Once all information is complete and correct, the authority will reply and provide the address for the submission of the original documents. -

Submit the application through the e-Submission system: access channel > https://privus.fda.moph.go.th > Business Operator > Submission of Exemption Request for Hazardous Substance Import (for Hazardous Substance Products) / System (F.12) (for Food Products) / Section 27 (Checkpoint) (for Medical Device Products) / Cosmetic Products System for One Specific Import (for Cosmetic Products).

Once approved, the system will issue an LPI number for the use pertaining to the issuance of import declaration by the Thai Customs.

*Documents may be submitted 1) at the One Stop Service Center, OSSC Building, Floor 4; or 2) to the email address bie.thaifda@gmail.com - once all information is complete and correct, the authority will reply and provide the address for the submission of the original documents.

Documentation

1) Invoice and other documents, as well as the product label information and picture applied to be imported.
2) Document detailing the health product applied to be imported **Food: comprising Health Certificate or Certificate of Free Sale or food receipt from the country of origin/ **Hazardous substance: Safety Data Sheet.
3Letter indicating the intention to import:
    - Name/number of the product applied to be imported;
    - Guidelines for the handling of product within 30 days after the filming is completed and an assurance of no selling/distributing/giving away, and that if there is any adverse effect resulting from the use of the product, the importer would take responsibility
   - Evidence/document confirming the destruction or the return of the product to the country of origin to be submitted to the Thai FDA within 30 days after the filming is completed. (FDA has a process for compliance with conditions in the process of granting approval)
4) Filming permit issued by the Department of Tourism.
5) Power of attorney for the coordinator from the company receiving the permit (which must be a person having residence in Thailand).

Processing Time

Within 24 hours for the review of the correctness and completeness of the application and accompanying documents in accordance with the rules and conditions prescribed by law.

Fees

    - Food: 2,500 Thai Baht in total (500 Thai Baht for the application fee and 2,000 Thai Baht for the reviewing of the application).
- Cosmetic: 100 Thai Baht for the application fee.
- Medical device and hazardous substance used in household: no fee.
